What is this service?

  • This is a certifying course that will provide participants with the necessary knowledge to conduct vision screenings of children ages three through 21 in a CHDP provider’s office.
  • This is a certifying course that will provide participants with the necessary knowledge to conduct vision screenings of children ages three through 21 in a CHDP provider’s office. Participants who complete the training must complete the post-test and the evaluation form in order to receive a Certificate of Completion. Recertification is recommended every four years. Please feel free to use all the resources on this page and save or print them for your reference. This training is intended for CHDP providers in San Benito County.
